repo definitions: where the names a file uses are defined #333

The blob page links a file's names to their definitions with one lookup (SymbolTargets) that no control command exposes, so a client drawing the file itself (the iOS app) cannot do the same. Add a read command returning, for each name a file uses that the index defines, the first definition and the count.
