Correct key hints in comments and the footnote message

The footnote jump named C-c &, which nothing binds; going back is C-c
C-c or C-c C-x f on the definition's label. MarkSubtree is bound to
C-c @ and the @ speed key, not M-h. The C-c C-c header now mentions
checkbox toggling.
