Make gitbay the canonical forge in code and docs
The instance list is fetched from gitbay, the About page reports issues
there, the index links the gitbay repository, and the help and
add-instance messages point at merge requests. The GitHub mirror stays
for the container image build and the README says so.
Closes #31
