web signup flow for open and invite instances #9

Registration is ssh-only even when the instance is open — a browser visitor has no signup path and view_only instances show no hint of how to join. In accounts mode with open/invite registration, a small form (username, email or invite code, paste an ssh public key) fronting the same registration path would close the loop.
